Step 1 – Make the Payment

Scan the QR Code displayed above or beside this form using any UPI application such as:

• Google Pay (GPay)
• PhonePe
• Paytm
• BHIM UPI
• Any UPI-enabled Banking App

Pay ₹200 towards the Elite Membership Fee for 1 year.

Step 2 – Note Your UTR / Transaction ID 

After the payment is successful:

1. Open your UPI application.
2. Open the successful payment details.
3. Copy or note the UTR (Unique Transaction Reference) or Transaction ID.
4. For Cash Payment, please mention Cash at UTR or Transaction ID

Important: Keep this number safe. It is required for application verification and future status tracking.

Step 3 – Fill the Registration Form 

Please ensure all information is accurate before submitting.

Step 4 – Submit Your Application

Click the Submit button.

Your application will be sent to the committee for payment verification.
